Medical, dental, vision, 401K and PTO available with no waiting period.rrrrrrrrrrrrrrResponsibilities and QualificationsrrrrrrrrrrPerforms personal care and other assigned duties to patients in their home under the guidance and supervision of the skilled nursing staff or therapy staff.ResponsibilitiesDrives from home to home to perform assigned tasks of personal care for each patient according to established policies and procedures. Assists patients with normal daily activities and responds to patient needs. This includes bathing, oral hygiene, skin care, and hair care. Takes vital signs.Performs procedures and treatments as assigned. Recognizes limitations of role and contraindications of treatment. Responsible for simple diagnostic procedures as assigned.Maintains clean and safe environment while performing patient care. Identifies need for safety measures and reports unsafe conditions.Documents all care and patient data accurately and submits documentation within the required time. Communicates with the nursing staff, therapy staff, supervisors, and other aides regarding case and scheduling concerns. Identifies and reports changes in the patients condition and takes appropriate action when needed. Participates in Performance Improvement activities, in-services, patient care conferences, workshops and seminars. Assist with orientation and training of other staff members.Attends and particicpates in interdisciplinary team meetings. Follows the established uniform policy. Presents a professional appearance, maintains a positive public image and a professional work environment and wears ID badge at all times.Qualifications:We’re all unique pieces of a puzzle at Mercy, connected together to create a beautiful picture of health. Qualification 2: Valid driver’s license.Current CPR thru the American Heart AssociationCertifications: CNA certified or have completed the Mercy Patient Care Tech trainingA combination of patient care experience and patient care training consisting of a minimum of 60 hours of classroom instruction plus a minimum of 16 hours of supervised practical training, in the competency areas specified in the Home Health Aide Competency Evaluation Check List.Preferred Other: Basic computer skills as well as a working knowledge of Windows, Word and Outlook.We’ll Support You at Work and HomeOur foundations are built on dignity and respect.
