For over 30 years, we have developed a culture that fosters innovation, clinical excellence, and integrity.Summary: The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient's needs. The home health aide is responsible for observing patients, reporting these observations and documenting observations and care performed. The home health aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care which includes provision of personal care and designated health- related services in compliance with agency's policies and procedures, applicable health care standards, and governmental laws and regulations. Responsibilities: • Providing personal care including:o Bathso Back rubso Oral hygieneo Shampooso Changing bed lineno Assisting patients with dressing and undressingo Skin care to prevent breakdowno Assisting the patient with toileting activitieso Keeping patient's living area clean and orderly, as appropriate• Planning and preparing nutritious meals.• Assisting in feeding the patient, if necessary.• Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency).• Assisting in ambulation and exercise according to the plan of care.• Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).• Assisting patient in the self-administration of medication.• Doing patient's laundry, as appropriate.• Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).• Reporting on patient's condition and significant changes to the assigned nurse.• Provides personal care, rehabilitative, support services, observation of patient through written and verbal communication.• Assists with quality improvement/quality assurance activities; records management as necessary.• Orienting new Home Health Aides when requested or required. • Assists in the implementation of a realistic, goal-oriented Plan of Care.• May perform additional tasks/procedures only if specific and adequate training has been given and documented by the supervisory Registered Nurse and allowed by state regulations, including providing nutritional support as needed.• Performs duties in accordance of and under the direction/supervision as defined by the Agency's organizational chart.• Follows agency policies and procedures. Maintains confidentiality of patients and agency information.• Participates in appropriate team conferences, staff meetings, in-service and committees with physicians and agency staff to ensure continuity of care.• Completes and submits all documentation, including clinical and progress notes in a timely manner according to agency policy.• Attends in-service and education as required.• Adheres to Federal/ State guidelines rules regulations, statutes and law.• Follows the Code of Conduct policy.• Complies with corporate and departmental policies and procedures. Requirements: Licenses/Certifications:•Meets the training requirements in accordance with state and federal laws. Home Health Aide training verification or CNA certification in the state of practice preferred.•Possesses and maintains CPR certification.•Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.•Active/current agency health requirements.•Must complete required department competencies to validate and maintain competency.Education/Experience:Education: Home Health Aide training verification or CNA certification in the state of practice preferred with HighSchool Diploma or equivalent depending upon state regulations.Experience: Prefer a minimum of three (3) months work experience as a Nursing Aide, preferably in home care, skilled nursing facility or hospital setting.If you have a passion for seniors, a special place awaits you on our Senior Advocacy Team!
