Performs resident care activities and related non-professional nursing services under thedirection of the Charge Nurse\/Nurse Manager to ensure quality care of residents.Essential Duties1. Provides individualized attention and nursing care in accordance with residentCare Plan which may include assistance with grooming, bathing, oral hygiene,feeding, incontinent care, toileting, colostomy care, prosthetic appliances,transferring, ambulation, range of motion, communicating, etc.2. Responsible for observing and promptly reporting any changes in residentconditions to Charge Nurse\/Nurse Manager (example: level of activity, eatinghabits, behaviors, and physical, mental and emotional status).3. Performs tasks assigned by Charge Nurse\/Nurse Manager including vital signs,residents' weights, applying non-prescription creams\/ointments, collectingspecimens, etc.4. Escorts residents to dining room, assists with feeding, and documents mealconsumption.5. Passes supplements, snacks, and fluids to residents per protocol and schedule anddocuments consumption.6. Provides care that maintains each resident's skin integrity in order to reduce therisk of pressure ulcers, skin tears, and other damage by changing incontinentresidents per protocol, turning and repositioning immobile residents per protocol,and applying moisturizers to fragile skin as needed.7. Maintains clean, neat and organized work environment.8. Protects the personal belongings of each resident including eyeglasses, dentures,hearing aids, furnishings, jewelry, clothing and memorabilia. Promptly reportsmissing items to Charge Nurse\/Nurse Manager per protocol.9. Answers resident call lights promptly and courteously.10. Assists with orienting residents and their families to the nursing home uponadmission.11. Escorts residents to appointments, including beauty shop, activities and churchservices and participates in activities and functions as directed.12. Completes certified nursing assistant electronic records documenting careprovided and other information in accordance with Facility policy and procedures.13. Participates in the training and orientation of new staff by demonstrating apositive, helpful, and enthusiastic attitude.14. Administers and accurately records the administration of medications forresidents as prescribed by the physician in accordance with established policiesand procedures.15. Observes, records and reports to Charge Nurse\/Nurse Manager symptoms andconditions of residents that could be related to medication interactions, adversedrug reactions or medication side effects.16. Determines and records effectiveness of medications in a timely and accuratemanner as required by state and federal regulations.17. Provides and promotes resident's rights during the administration of medications.18. Stores medications in a safe and accurate manner in accordance with establishedFacility policies and procedures of and applicable state\/federal regulations.19. Monitors medications to ensure adequate accountability measures are taken whenmedications are ordered, received into the Facility, administered and at change ofshift as required by policies and procedures of Facility and applicable state\/federalregulations.Standard Requirements1. Cooperates and works together with all co-workers; plans and completesjob duties with minimal supervisory direction, including appropriatejudgment.b. Uses tactful, appropriate communications in sensitive and emotionalsituations.c. Follows up as appropriate with supervisor, co-workers or residentsregarding reported complaints, problems and concerns.d. Promotes positive public relations with residents, family members andguests.e. Completes requirements for in-service training, acceptable attendance,uniform and dress codes including personal hygiene, and other workduties as assigned.f. Participates as assigned in the orientation of new staff and demonstrates apositive, helpful, and enthusiastic attitude.6. Immediately reports incidents of alleged resident abuse or neglect or allegedviolations of residents' rights to Director of Nursing or Executive Director.7. Knowledgeable of age specific developmental factors specific to adult andgeriatric residents (i.e. physical, cognitive and socialization factors) in planningdelivery of care.8. Maintains current practice standards or changes by participating in educationalopportunities provided by the Facility and outside resources.Other DutiesPerform tasks which are supportive in nature to the essential functions of the job, butwhich may be altered or redesigned depending upon individual circumstances. Performsother work duties as assigned.QualificationsEducation\/Training: A high school diploma or equivalent preferred. Must havesuccessfully completed State approved course work and examinations to obtaincertifications as both Certified Nurse Assistant and Certified Medication Technician.A.D.A ((Americans with Disabilities Act)(as amended)) REQUIREMENTS: Must beable to bend, stoop, push, pull, and walk and stand for most of the working day. Requiresmanual dexterity sufficient to provide hands-on care to residents as well as to operatestandard office machines such as fax, telephone, etc. Requires normal range of hearingand vision. Must be free from communicable diseases that pose a \direct threat\ to thehealth or safety of others and which cannot be eliminated by a reasonableaccommodation. Must be able to lift a minimum of 50 pounds, and transfer and transportresidents. Must be able to carry and transport supplies. Must be able to pass a post-offer,pre-employment physical health examination as a condition of employment. Must becapable of performing the essential functions of the job, with or without reasonableaccommodation(s).Skill(s): Proficient reading, writing, grammar, and mathematics skills; proficientinterpersonal relations and communicative skills; knowledge of drug interactions, sideeffects, and incompatibilities; decision-making skill; proficient assessment skills; basiccomputer skills; safety techniques; able to operate various medical and office equipment,auditory and visual skills. Must be able to communicate in English, both verbally and inwriting, and possess sufficient communication skills to perform the tasks required.Experience: A minimum of three months' prior nursing assistant experience in long-termcare preferred.
