Home Health Aide/HomemakerThe Home Health Aide/Homemaker provides personal care and light housekeeping services to patients in the home, nursing facility, inpatient units or other environments where they reside, in accordance with an established plan of care and under the supervision of a registered nurse. The Home Health Aide/Homemaker observes the patient, reports and documents observations and care performed. Provides timely documentation of services based on the patientâ?'s care plan. As specifically assigned by the designated supervising primary care nurse, the duties of the homemaker-home health aide may include: assisting the patient with personal care activities including bathing, oral hygiene, feeding and dressing; assisting the patient with exercises, ambulation, transfer activities and medications that are ordinarily self administered; and performing normal household services essential to patient care at home, including shopping, meal preparation, laundry and housecleaning.Personal Care and Homemaking Services: Ensure the quality and safe delivery of personal services to the patients while also supporting their families and caregivers. Perform or assist patient with personal care activities as prescribed in the care plan, including but not limited to hygiene, mobility and elimination. If unable to perform a certain task, reports to the RN or Director of Clinical Services immediately. Provide and maintain safe environment for patient, by using equipment safely and properly such as footstools, side rails, oxygen, etc. Practices universal precautions in all contacts with patients. Perform or assist patient/family with home maintenance activities as prescribed in the care plan including, but not limited to light housekeeping and meal preparation. Perform support activities as prescribed in the care plan including but not limited to supporting independence, providing companionship, and providing respite for families. Respect the autonomy and right to confidentiality of all patients and families. Monitor adequate fluid and nutrition intake, temperature, pulse, respiration, skin condition and basic elements and changes in body functioning and record/reports appropriately as prescribed in the patientâ?'s plan of care. Practice safe transfer, ambulation, normal range of motion and positioning techniques. Document care provided and response of patient/family to care and submit documentation in a timely manner in accordance with agency policies. Work in cooperation with family/caregiver/and other home health staff to meet the emotional needs of the patient/family/caregiver.Human Relations/Contact and Communications: Must maintain a professional attitude and demeanor and is be able to communicate effectively and relate courteously and cooperatively with patients, caregivers, colleagues, supervisors, co-workers and all others. Must be able to demonstrate optimism, enthusiasm and willingness to work constructively with other health care team members and reflect concern for the well being of patients and employees. Interacts with all staff in a positive fashion supporting the mission and vision of the organization.Qualifications:Education:· High school diploma or equivalent.· Completion of a state approved training program and certification as a home health aide/homemaker, and proof of certification.Experience:· Minimum age requirement of 18.· Minimum of 1 year of direct patient care experience in an institutional or home care setting required with community health experience preferred.· Ability to work within an interdisciplinary setting.· Understanding of home health philosophy and needs of the home care patient.Licensure, Registration, and/or Certification:· Valid driverâ?'s license and reliable transportation that is insured in accordance with Connecticut requirements.· Satisfactory references from employers and/or professional peers.· Satisfactory criminal background check.
