Provides in-home care services to older adults, according to their established plan of care. Under the direction of their assigned supervisor, performs or assists with bathing, grooming and other personal care needs; assists with grocery shopping and other essential errands; performs routine housekeeping and laundry; assists with meal planning and meal preparation; provides medication reminders. Must be willing and able to travel to various locations within Chicago and surrounding areas, as needed. To be actively involved and knowledgeable of the Agencyu2019s Performance Quality Improvement process. Approves time sheets (calendars) and ensures compliance with agency and funder guidelines. Accountable for daily activities within the allowable activities approved by the Agency.u00a0 Essential Duties: Include the following, other duties as assigned: Responsible for maintaining a positive and professional relationship with participants, in order to support their independence and a high quality of life, as much as possible. Provides in-home care services, according to the established plan of care. Assists with meal planning and engages in meal preparation, as needed. Performs routine and/or light housekeeping (e.g. making and changing beds, dusting, washing dishes, vacuuming/sweeping, cleaning/mopping floors, cleaning kitchen and bathroom, and doing laundry. Performs/assists participants with local grocery shopping and/or other essential errands; including maintaining all required receipts and documentation. Provides self-administered medication reminders: reminding the participant to take his/her medication; reading instructions for utilization; uncapping medication containers and providing the proper liquid and utensil with which to take medication. Assists with personal care tasks (e.g. shaving, shampooing/brushing/combing hair, shower/tub, or sponge bathing, dressing, oral hygiene/teeth or denture cleaning, preparation of appropriate supplies, transferring participant, and assisting participant with range of motion needs. Assists with and follows written, special diet plans and reinforces nutritional maintenance-only as indicated by the plan of care and/or by the primary physician. Accompanies participants to appointments via public transportation or other community sources (e.g. taxi cab, Lyft, Uber, or Medi-car), if required by the established plan of care. Maintains service schedule record /daily calendars, as required. Monitors participantsu2019 progress, including any concerns, changes or issues regarding the participantu2019s emotional, cognitive or physical functioning, to ensure the participantu2019s ongoing safety and well-being. and informs the assigned supervisor in a timely manner. Reports other relevant concerns, changes or issues related to the participantsu2019 home environment or life situation to the assigned supervisor in a timely manner. Must attend initial pre- service trainings and ongoing continuing education trainings, staff meetings, etc. as required. Must inform the assigned supervisor immediately, of any situation that will affect participantu2019s service delivery, including obtaining prior approval by assigned supervisor in regard to any changes to be made in a participantu2019s established service schedule or the tasks being performed. May be required to perform other duties, as assigned by supervisors and/or program director. General Requirements:u00a0 Sensitivity to cultural diversity is required. Education/Experience: A minimum of high school diploma or general education diploma (GED) strongly preferred; or at least one year or more experience serving as a homemaker or caregiver; experience working with the older adult population is also preferred. Language Skills:u00a0 Must be bilingual (English & Spanish), unless assigned as a family worker, with good oral and written communication skills. Ability to read, write and comprehend instructions, short correspondence and memos including documents such as safety rules, training curriculum, and program or agency manuals. Mathematical Skills: Must possess basic math skills in adding and subtraction. Understanding whole numbers and whole number operations. Operating Environment and Working Relationships:u00a0 Must maintain positive and respectful relationships with older adult participants and their family members at all times. Must demonstrate patience and a good emotional disposition in order with the older adult population and individuals with varying level of need. Must possess the ability to deal tactfully with colleagues, supervisors, and participants and their family members. Physical Demands:u00a0 The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
