High school diploma or equivalency

Well developed verbal and written communication skills

Access to reliable non-public transportation with required insurance coverage

Flexibility in scheduling

Ability to operate computer equipment; knowledge of various software packages including Office Windows applications 2007 and above

A. Serve as an advocate and assist individuals in understanding options and in communicating their choices regarding living arrangements and work. Activities include inform participants of activities and events, employment opportunities, act as an appropriate role model, maintain professional relationships with family, county staff, and Aptiv team members, notify stakeholders of pertinent information relating to the participant.

B. Deliver quality personal care service. Activities include following care plan, assisting with transition to and from on-site, facilitating assisted ambulation, toileting, various methods of transfer, meal set up and preparation, assisting with eating, tube feeding, and oral hygiene, spot bathing, dressing, and assistance with daily routines.

C. Assist participants in building natural supports and becoming integrated into their community environments. Activities include promote the development of friendships in their neighborhood; provide education to community; assist in the development of recreation interests, and connect the participants with people of similar interests.

D. Complete all required documentation in a timely manner. Activities include daily/weekly/or monthly updates on participant contacts and progress, time and billing sheets, job analysis, standardization of tasks; evaluating worker through the use of wage determination, record the results of the wage determinations, and all other pertinent documentation to ensure current case file.

E. Deliver quality residential services. Activities include follow development plan goals, support the participants in community integrated skills (cleaning, cooking, menu planning, grocery shopping, laundry, manage personal finances etc.); systematically fade support; conduct job analysis of newly developed work sites, provide written job analysis and work schedule; problem solve, educate participants in safety issues, administer first aid when applicable, assist with personal care needs.

F. Monitor participant's general health. Activities include support the individual in following physician recommendations, accompany the participant to various appointments (i.e.-medical, hair, dental, job interview), advise concerning personal hygiene issues, and report any concerns to supervisor and other team members in a timely manner.

G. Provide transportation services. Activities include provide transportation for participants when necessary, encourage and assist in the development of knowledge in utilizing other forms of transportation , educate and monitor pedestrian safety skills, and provide assistance with participants who use wheelchairs.

H. Maintain high performance standards. Activities include follow organizational policies and procedures; attend team meetings; maintain job knowledge through training and orientation; provide quality customer service; maintain solid working relationships, manage time, use resources wisely, and maintain solid working relationships, confidential, and ethic standards.

I. Maintain current knowledge and assure compliance with applicable laws, rules, regulations, Aptiv policies and procedures, and state and national standards. Activities include periodic review of Aptiv policies and procedures and state and national standards; and maintain and practice all aspects of safety while performing duties.

J. Must be capable of working a flexible schedule. Activities include rotating/split shifts, weekends, and holidays with the ability to transition between numerous diversified living and work sites.


Work time will be spent in the home, community or work setting assisting the participants in achieving their development plan goals. Mobility within the rural and urban community is required on a daily basis. Occasionally may push/pull wheelchairs and assist with transfers in or out of wheelchairs for adults of various sizes. Occasionally required to sit at a desk or in meetings for an extended period of time. Ability to present information and respond to questions from participants, staff, and the general public when speaking on the phone, corresponding on the computer or in face-to-face interactions.

Typical sensory activities may include jumping, running, swinging, and close physical contact.

Lifting weights of up to 50 pounds regularly, and up to 20 pounds daily, required at some work sites. Lifting requirements may change according to each work site. Bending and walking required regularly.

Uses hearing and vision with the ability to collaborate and orchestrate in a variety of environments. Regular use of fine motor skills for writing or typing.

Regularly will concentrate on multiple tasks simultaneously along with the capacity to respond calmly and quickly to emergency situations required. Ability to interpret an extensive variety of technical instructions and deal with several abstract and concrete variables. Capacity to differentiate between various levels of confidentiality and act accordingly.

Ability to obtain first aid certification, based on program requirements, within 30 days of hire.


Work is performed in the home, community, and work settings with varied conditions. Outdoor travel required on a daily basis.
